What is Community Nursing Care?

Community Nursing Care is clinical support provided by qualified nursing professionals outside of a hospital setting—typically in your home or another community location. This support is ideal for people who need ongoing or short-term nursing intervention due to health conditions, disability-related needs, or post-hospital recovery.

Our nursing services are available to eligible NDIS participants with funding under Capacity Building – Improved Daily Living or Core Supports, depending on your situation. If you’re unsure which category applies to your care, we’re happy to assist you with navigating your plan and funding options.

We Will Assist You With

In-home care empowers freedom and enhances quality of life by offering clients individualised support and compassionate care in the comfort of their own homes.

Wound Care Management

We assess, treat, and manage a wide range of wounds, including post-surgical wounds, diabetic ulcers, and pressure injuries. Our nurses use appropriate dressings and evidence-based practices to promote healing, reduce pain, and prevent complications.

Medication Management

We assist with safe and timely medication administration, including oral, topical, and injectable medications. We can also organise Webster packs, monitor for side effects, and liaise with your doctor for medication reviews or changes.

Continence and Catheter Care

Our nurses offer respectful continence support, including catheter changes, stoma care, and bowel programs. We ensure comfort and hygiene while helping to reduce the risk of infection and improve confidence.

Chronic Disease Management

We offer ongoing support for managing chronic conditions such as asthma, COPD, heart disease, and neurological disorders. Our care plans are designed to help stabilise symptoms and improve your quality of life.

Enteral Feeding (PEG and NG Tube Care)

We provide skilled support for clients with PEG or nasogastric tubes. Our services include feeding assistance, site cleaning, tube changes, and monitoring for signs of blockage or infection.

Post-Hospital Discharge Care

We support your recovery after hospital discharge by reviewing care instructions, managing dressings or medications, and monitoring your progress. Our goal is to reduce the risk of readmission and support a smooth transition back home.

Diabetes Management and Insulin Support

We help manage all aspects of diabetes, including blood glucose monitoring and insulin administration. We also provide education to support self-management and coordinate care with your GP and allied health professionals.

General Health Monitoring

We provide regular health checks including vital signs, weight monitoring, and general wellbeing assessments. This allows us to detect any changes early and keep your care team informed and up to date.

Palliative and End-of-Life Care

We provide gentle, respectful nursing care for clients with life-limiting illnesses, supporting comfort, dignity, and quality of life. We work alongside families, GPs, and palliative teams to manage symptoms and honour individual preferences.

We provide Community Nursing Care to NDIS participants who require clinical support at home. You may be eligible for this service if you:

If you’re not sure whether your NDIS plan covers nursing care, we’re more than happy to guide you. We can work with your support coordinator, help gather reports from your treating professionals, and assist in applying for the right funding category.

A Collaborative Approach

We don’t work in isolation. We know that the best care happens when everyone is on the same page. That’s why we keep open communication with your GP, specialists, support workers, and allied health providers to ensure your care is well-rounded and coordinated.

We also provide written updates, health reports, and clinical documentation to support plan reviews or medical referrals when needed. Our team can even assist during times of change—like hospital admissions or discharge planning—so you’re always supported.
